Context & Root Cause
Why a single Up arrow didn't show up: Carnac shipped with konami.yml enabled by default (Up, Up, Down, Down, Left, Right, Left, Right, B, A). Because the Konami sequence starts with Up, Up, Carnac's ShortcutAccumulator treated your first Up arrow press as the beginning of a potential multi-key chord and buffered it indefinitely waiting for the next key. Only after pressing Up a 3rd time did the chord fail to match and flush as ↑ x 3.
The Fix:
Moved konami.yml to an opt-in sample (Keymaps/samples/konami.yml.sample) so normal Up arrow key presses are never hijacked out of the box.
Added a 1-second chord timeout (ShortcutAccumulator.Flush()) in MessageProvider so that even when multi-step chords (like Ctrl+K, Ctrl+C in Visual Studio / VS Code) are used, pressing the first key alone will automatically flush and display on screen after 1 second instead of being swallowed forever.
Upcoming Release: This will be included in the next official release, which will be published to GitHub Releases (both installer and portable .zip), WinGet (winget install doggy8088.Carnac), and Chocolatey.
